Every org I scan has duplicate accounts. Every single one.
The average? 10-15 duplicate account name groups in a 5,000-account org. That doesn't sound like a lot until you realize what it means:
Salesforce has duplicate rules, but they only fire on create. If your duplicates were imported, migrated, or created before the rules existed, they're invisible.
And fuzzy matching ("ACME Corp" vs "Acme Corporation") isn't something standard duplicate rules handle well.
